To save time, it would be good if you could complete your Client Information Form and bring it with you to your visit.

To Bring to your first appointment:

Remember to bring your medicare card to your first appointment.

Also, could you bring your current glasses if you have any.

If you wear contact lenses, then could you bring the box or blister pack that your contact lenses came in.
